Will a 20 KB photo still look good?

The tool first lowers the picture size only as much as needed, then picks the highest JPEG quality that stays under 20 KB. 20 KB is very small: roughly a compact passport-style photo of a couple of hundred pixels across. The tool only shrinks the picture as much as it has to.

Can I also set an exact size like 200×230 pixels?

Yes. Open "Exact dimensions", enter the width and height, and choose "Crop to fill" or "Fit inside on white". The result is still kept under 20 KB.
